Katherine can be contacted through her confidential voicemail.  Upon returning your call she will discuss her availability and any questions you may have.  Please leave the number to a confidential voice mail where she can leave a message if necessary. 

Fees

Payment is due at the time of service.  This can be through your insurance plan co-pay or paying my full rate out-of- pocket.  My fees for a therapeutic hour are $105.00.  If you plan on using your medical coverage for counseling you must contact them prior to your first appointment to confirm your coverage:

  1. Is Katherine Bogushefsky,MC ,LPC, an in network provider on their plan?
  2. Do you have a deductible to meet before your coverage begins?  How much?
  3. If you pay co-pays, how much, and for how many visits?
  4. Is there a preapproval code needed to start counseling?
  5. Do you have any free (EAP) visits?  How many?
  6. Is there a preapproval code for the EAP visits?
  7. Are there any issues or diagnoses your behavioral health won’t cover?

If Katherine is not an in network provider you will be responsible for her standard fee. She can provide clients a “Super Bill” to be submitted to their insurance companies.  The insurance company will refund what their coverage allows. 

Appointment Cancelation Policy

Because a therapist cannot bill the insurance company for services not provided, Katherine requests that if you are unable to attend an appointment you cancel within one full business day. (24 hours, Monday through Friday)  This enables her to find a replacement for your scheduled appointment.  

Session Length

There are sometimes misunderstandings about the length of sessions.  Therapy sessions, as defined by the American Medical Association Current Procedural Terminology coding are 45-50 minutes.  This is known as a “therapeutic hour”. 

Telephone Counseling

Even though I have a preference for face-to-face counseling, there may be times when some telephone counseling is warranted.  In those times be aware that insurance companies and managed care organizations generally do not reimburse for these services.  Telephone counseling should be scheduled for a mutually-agreeable time and will be billed at 37.50 for each 15 minute period of counseling during normal business hours.  Evenings, nights, and weekends are billed at an additional premium.

Insurance Warning

  • Please be aware that when utilizing insurance plans for counseling services, your privacy is compromised. This is especially important to be aware of when dealing with issues of a sensitive nature.
  • Insurance companies also generally only cover in-office visits, therefore, if you are interested in telephonic or virtual appointments, please contact your insurance company to verify coverage.
  • Many times insurance companies give prior authorization for treatment with the stipulation they have the right to refuse payment. As a client, you are responsible for any unpaid service fees.
